I asked for my MAC code two weeks ago and was told I would have it by email within two days. Why could I not be given it while I was no the phone and WHY have I STILL not been sent it?.
The agent has to raise it with usually customer care, and the code is requested then and emailed to you. The code isn't immediately available. (I think the code actually comes from BT, i'm not 100% on this)
as for the actual delay, then there is an issue that needs escelating..

Do we have to go to Ofcom each time?
I know how you feel sort of I did get my MAC in 48 hours but because the new ISP tried to connect me at a speed my line couldn't support then I need another and OFCOM have told me that once a code is used my ISP can issue another straight away but the call centre insist that I have to wait 30 days. About 1 hour ago I sent an email to the ISPA as I do not believe they cannot generate another MAC.
