I don't see the point in Orange or any company botheringing with LLU to upgrade to 8megs (ha ha) when pretty much every exchange supports 8meg anyway, why on earth do Orange waste money and time installing their own sh***y euipment when it's not needed, by the time they eventually get everyone on an upto 8meg, BT exchanges will be 24meg ready.
They have a greater degree of control over what services they can provide.
Certain services require an exachange presence or very deep system/equipment access to be able to provide, such as the BT Vision.
Depending on the equipment used, many ADSL Max systems can also supply ADSL 2+ speeds simply with configuration changes. However the main reason Orange don't do ADSL 2+ is the livebox can't support them so a new router will be required. There's long been talk of a livebox 2 which would presumably support these speeds along with other integrated features. The livebox features when it first came out were pretty advanced for the time.

When the livebox first came out ADSL 2+ hadn't even been implemented anywhere comercially AFAIK. It was a very new standard and as the industry moves so fast standards wise it's likely they didn't want to spend money supporting it when it may just be superceded by something better and cheaper.
Also according to documentaion on the original inventel router it's based on it appears the broadband connection is limited to about 13meg for some reason.
Are you sure that it's more expensive using llu rahter than BT wholesale, because adsl max is now pretty much standard in 99% of exchanges, even if it does cost more, i bet Orange lose far more money by losing customers and compensating (i'm assuming they have some decency mind!) for such a poor service
